It is the responsibility of the student seeking review to present his or her case to the hearing board.

Instructions:

  1. Fill out all requested information on-line.
  2. Submit the form.
  3. Print or save a copy for your records.
    • You will receive a copy of this form e-mailed to the address you provide in the "E-mail" field.

Important note about appeals:

An appeal is a review of a previous decision, in this case the decision of the responsible faculty member.   While you will be allowed an opportunity to address the hearing board and conduct a defense, the scope of the review is limited the findings and conclusions provided in the written decision from the faculty member.  If you have questions, contact the Office of Student Conduct before submitting the appeal.

The duties of the academic integrity hearing board are as stated in WAC 504-26-404(2)(b):

(b)    The academic integrity hearing board is empowered to provide an appropriate remedy for a student including arranging a withdrawal from the course, having the student's work evaluated, or changing a grade where it finds that:

(i)  The student is not responsible for violating academic integrity policies; or

(ii) The outcome imposed by the instructor violates the instructor's published policies.
